"I've had saline implants for 10 yrs (on top of muscle). I have bad rippling and am considering new implants. I consulted with a surgeon and he recommends silicone implants (on top of muscle). He says because silicone is firmer than saline implants, this should take care of the problem. I'm a bit nervous about silicone hearing that it does not look as natural as saline. What is your recommendation?"

Replacement with silicone is the best option. You may consider placement under the muscle which will additionally help hide the implant on the upper pole. If you want to stay with saline then you could have a newer type that are more overfilled and have less wrinkling and have them placed under the muscle. Also if your implants are textured on the surface, then replacement with smooth-walled ones, either silicone or saline, should have less wrinkling.
